4/25/92 (Day 161) — Children’s Memorial Day 13

Our new best friend. Even if it only gave Ike a few minutes of comfort, that was better than nothing. The poor nurses had Isaac all day. I spent the day at Childrens, trying to figure out when they were going to release Molly.

Home nursing notes:

Arrived midnight and got report from dad, who stated: “Isaac’s been up since 9 o’clock, he’s been fussy. Took 10 ccs at 10 o’clock and another 10 ccs at 11.” Dad was holding Isaac, who’s color was pink, and skin was warm and dry. He calms down when sucking on his pacifier. I swaddled him in his blanket and he took 25 ccs 1/2 breast milk 1/2 formula with good suck/swallow coordination. Burped then fell asleep after the feeding. He woke crying at 2:50 for a diaper change and took 50 ccs of 1/2 and 1/2 with red nipple over 30 minutes. Lips dusky during burping. Hands cool to touch, torso warm. Mild respiratory increase and retractions during feeding. Swaddled and slept in crib.

Woke crying at 5:00 a.m. Attempted to bottle feed after diaper change; he took 5 ccs eagerly before starting to fall asleep. He burped well, then sucked on the bottle lightly without ingesting anything. I held him for 15 more minutes, swaddled him and put him back in the crib at 6:00. He woke at 7:10, cried frequently during feeding and burped four times. Took 35 ccs. Turned dusky during burping. Swaddled and placed in crib, where he cried intermittently.

I gave a.m. meds except for Mylicon. Ike slept pretty well, crying often after the 7:00 a.m. feeding. Dusky during burping and crying with two of his feedings, but pink when nippling and not crying. There’s a bottle in the refrigerator that only has about 40 ccs in it — I used some from it for his last feeding. Have a good weekend. — Joann

11:00 a.m. Received from parents swaddled in swing. Crying intermittently and sucking and biting pacifier, slightly dusky when crying. Between 11:30 and 12:30 took 35 ccs of 1/2 breast milk 1/2 formula. Clamps down frequently on nipple when feeding. Had a larger, loose green stool @ 1:00 p.m. Irritable, crying continuously unless being held with pacifier in his mouth. Sleeps for very short intervals (<10 minutes) while being held, but then awakens crying. Color pink, but dusky when crying vigorously. Dad states not to wash Isaac or change his clothes, as that was done this a.m. At 4:00 awake and crying; gave meds ad he took 35 ccs. Carried in Snuggli. At 5:30 awake and crying continuously. Aerocyanosis noted when crying. At 6:00, took 25 ccs, very irritable, crying unless being held or in swing.

9:00 p.m. awake and fussy. Took 60 ccs before being returned to crib.

    I remember the days when my twins had to have mylicon….ended up having to get a stronger form from the doctor but was SO glad to have it! : )

      I’m not entirely sure it ever really worked for Isaac (maybe for short periods right around feedings), but at least it felt like we were trying to help him. He was so uncomfortable. My brother — who was not a preemie — never learned the suck, swallow combination and since my mother didn’t nurse, it took months to figure things out. He would howl and take forever to eat, then completely wear himself out. As soon as she was able to switch him over to a cup, everything was fine.
