5/11/92 (Day 177) — Preemies at Home, day 44

Here's Ike in a relatively quiet moment, strapped into his bouncy seat and set in his crib. He got to hang out with the nurses at home while I took Molly downtown for her follow-up visit with the surgeons. Whenever possible, we avoided bringing them anywhere near sick people, especially the hospital. All I really remember about that follow-up visit was a lot of congratulating by the two surgeons of the work they had done, and then them dismissing Molly from their care forever. Can't say I really miss them, although I should probably congratulate them on a job well done, too.

Home nursing notes:

Isaac

Ike received from parents prone in crib at midnight. Awake for diaper change and feeding at 3:00 a.m. Asleep after nippling 20 ccs, returned to crib prone. Awake and crying at 5:00 a.m., nippled 50 ccs with good burps. Awake and crying at 6:00 a.m., nippled 35 ccs, burped well and returned to crib prone. — Tom

Awaking in crib at 8:00 a.m. took 20 ccs formula, we diaper changed. Took 35 ccs formula at 9:30, diaper changed and returned to crib prone to sleep. Awake and hungry at 10:40 a.m., took 25 ccs formula and burped. Axillary temp 98.4°. Clothes changed. Apical pulse 140, respiratory rate 72. Returned to crib at 11:10 asleep on tummy. Awake at 12:30,  took 30 ccs formula. Fell back to sleep immediately. Awake at 1:30 p.m., too additional 30 ccs formula. Irritable while sucking. No burps. Gave Mylicon per order. Back to sleep at 1:45 p.m. Awake at 2:30 after 45 minutes sleep. Took 35 ccs formula. Mushy brown stool; changed diaper. Continues to be irritable at 3:30, supine in crib watching mobile. Saturating at 98%. — D. 

Molly

Received asleep at midnight. Awake at 3:00 a.m., diaper changed, up for feeding, nippled 10 ccs. Awake at 5:00 a.m. for diaper changed and feeding, nippled 50 ccs. Offered second bottle at 6:00 a.m., nippled 50 ccs formula. Returned to crib prone after burping. Awake and crying at 7:00 a.m. Nippled 20 ccs; returned to crib prone. — Tom

Took 15 ccs at 8:00 a.m. Changed liquid stool diaper at 9:00 a.m. took additional 10 ccs formula and burped; placed in crib. Nitropaste changed at 10:00 a.m. to L arm per order. Changed wet diaper and clothes. Took 15 ccs formula. Axillary temp 97°F. Went to doctor visit with mom at 10:45 on portable oxygen. Returned from appointment at 1:00 and reconnected to O2 tank in room at .25 Lpm per NC. Mom reports too 20 ccs formula while gone. Nitropaste treatment applied at this time. Given Lasix at noon as ordered. Took additional 25 ccs, burped. Had large yellow liquid stool. Irritable at 2:00 p.m., took 25 ccs formula. Back to crib on tummy with pacifier at 2:30. Sats at 99%. Left in mom’s care at 4:00 p.m., sound asleep in crib. — D.

From the heart surgeons to the cardiologists:

Thank you for referring your patient, Molly Bearman, to Children’s Memorial Hospital and our cardiac surgical group. She was discharged May 4, 1992 after pericardial patch closure of atrial septal defect and ligation of patent ductus arteriosus. Her postoperative course was marked by prolonged ventilatory support secondary to her pulmonary hypertension, with extubation on April 23rd. She was discharged on the following medications: Lasix, 2 mg PO b.i.d., Nitropaste, 2 cm  q 2 hrs, and oxygen via nasal cannula at .25L/min. Weight at discharge was 2.56 kg.

The parents have received instructions on wound care and were advised to call us in the event of any difficulties. We will see Molly within two weeks for follow-up. We advise the usual recommended American Heart Association antibiotic prophylaxis as it applies to your patient.
